近日，發(fā)表于《風(fēng)濕病學(xué)年鑒》的一篇論文對(duì)類風(fēng)濕關(guān)節(jié)炎(RA)與勃起功能障礙(ED)相關(guān)性進(jìn)行了研究,，研究表明,，男性RA患者發(fā)生ED的風(fēng)險(xiǎn)較非RA患者高出2/3。
既往研究提示,，RA患者可能更易于發(fā)生其他疾病,，并與心血管風(fēng)險(xiǎn)增高和慢性炎癥相關(guān)。但至今為止,，尚無(wú)研究探索ED與RA之間是否存在關(guān)聯(lián),，為此臺(tái)北醫(yī)科大學(xué)的Joseph J Keller醫(yī)生和Herng-Ching Lin博士對(duì)二者之間的相關(guān)性進(jìn)行了研究,。
這項(xiàng)研究納入2001~2009年間首次診斷ED的6,310例患者(年齡18~80歲)作為病例，并納入年齡(以10歲為間隔)和索引年份與之匹配的37,860例對(duì)照患者(每個(gè)病例有6個(gè)對(duì)照),。僅納入2001~2009年間至少2次被診斷為ED的患者(至少有1次是由泌尿科醫(yī)生做出診斷),，以提高ED的診斷準(zhǔn)確性。研究者還選擇了在索引日期前至少有2次被診斷為RA的患者,，并且至少有1次是由風(fēng)濕免疫科醫(yī)生做出診斷,。此外，僅納入至少被處方1種緩解病情抗風(fēng)濕藥物的RA病例,。該研究中使用了臺(tái)灣縱向健康保險(xiǎn)數(shù)據(jù)庫(kù)2000年的管理性索賠數(shù)據(jù),，該數(shù)據(jù)庫(kù)中包含臺(tái)灣國(guó)民健康保險(xiǎn)計(jì)劃中100萬(wàn)人的所有醫(yī)療索賠和注冊(cè)數(shù)據(jù)。
結(jié)果顯示,，校正月收入,、地理位置、城市化水平,、高血壓,、糖尿病、冠心病,、高脂血癥,、肥胖和酒精濫用/酒精依賴綜合征等可能的混淆因素后，條件性Logistic回歸分析顯示,，RA患者發(fā)生ED的風(fēng)險(xiǎn)率是對(duì)照者的1.67倍,。)

A population-based study on the association between rheumatoid arthritis and erectile dysfunction

Increased mortality in rheumatoid arthritis (RA) patients can be partly attributed to an accompanying increase in cardiovascular risk. On account of the increased cardiovascular risks and chronic inflammation, patients with RA may be more likely to contract other conditions with shared risk factors, such as erectile dysfunction (ED).
